- IDPH conducts an initial review of the application and contacts the applicant for revisions by email. 30 days
- The application must be completed and approved within 90 days of submission or it may be denied under Section 245.130(b). 90 days
- IDPH will issue a provisional license to a new agency within 90 days after receipt of a compliant application. 90 days
